The USDA will NOT ACCEPT THE KENTUCKY EIA form after April 15, 2020.
Approved paper EIA forms are the:
- VS10-11 Feb 2018 edition
- Ohio Department of Agriculture
- For all paper forms, every box on the form has to be filled in, including a tattoo or brand number. If there is no data to enter, must draw a line or indicate “none”
- Approved electronic forms are Global Vet Links and VSPS.
